    ASSISTANCE ON THE GRAWE DOCTOR PROGRAM AND CORPORATE HEALTH INSURANCE PROGRAMS

    ACTIONS IN THE EVENT OF AN INSURANCE EVENT

    Settlement of insurance cases is carried out directly through the Insurer's Representative of the Assisting Company, which resolves the issue of the necessity and procedure of providing medical services to the Insured Person within the scope of the Insurance Program.

     

    In the event of an insured event, you must:

    • call medical assistance immediately

    Report the following data:

    • Insurance contract number

    • Surname, first name of the Insured Person

    • A detailed description of the circumstances and nature of the required assistance

    • immediately, but no later than 24 hours after the occurrence of the insured event, seek help from a medical institution and perform all diagnostic and treatment procedures prescribed by the doctor, as well as seek help from the competent authorities (according to the nature of the event: competent units of the Ministry of Internal Affairs, ambulance service, fire protection authorities, gas service, etc.);

    • within 2 working days from the date of occurrence of the event, notify the Insurance Company "GRAWE UKRAINE" about the occurrence of an insured event (by means of telephone communication, e-mail) indicating the surname, first name, patronymic of the Insured person, contract number, reasons and circumstances of the insurance event, consequences of the insured event (if they are known);

    • within 3 working days from the date of the occurrence of an event that has signs of an insured event, submit a written statement-notification of the occurrence of an insured event to PJSC "Grawe Ukraine Insurance Company";

    • in advance, but no later than 30 calendar days in advance, to notify the GRAVE UKRAINE Insurance Company of your intention to submit medical documents to the medical and social expert commission (MSEK) for the purpose of assigning a disability group.

     

    Actions of the Insured Person in case of absence from the city in the event of an insured event of medical facilities with which the Assisting Company cooperates:

    • Notify the Assisting Company about the occurrence of an insured event and follow its instructions. In particular, if there are no medical facilities in the region (city, village) with which the Assisting Company cooperates, the Insured Person may be asked to independently call an ambulance at number 103, which will direct the Insured Person to the next medical facility as a resident of the city. In such a case, the Assisting Company undertakes to provide the Insured person with medicines during treatment in such a medical institution under the terms of the contract. If the medical institution in which the Insured Person is hospitalized is unable to provide the necessary treatment and according to the opinion of the doctors, the Insured Person is transportable - the Assisting Company ensures the organization of transportation of such a person to another medical institution.

    • If it is impossible to contact the Assisting Company - independently contact medical institutions whose services are covered by the insurance contract, depending on the Program, with further notification of the occurrence of an insured event to the Assisting Company and/or the Insurer.
